Instructions for Adobe Acrobat Reader Users
All our forms are interactive, which means you may fill them out on your computer. Simply double click on the form(s) to open, and then click in the spaces and boxes to enter the information. When completed just click on the print button and sign. Bring in your signed form(s) along with your pet’s medical records prior to or at check-in. You will not be able to electronically save the forms to your computer. If you prefer to hand write out the form(s) simply open the form(s) and click on the print button.
Note to Adobe Acrobat Full Version Users:
You should be able to digitally sign and save your filled out forms to your computer (save as .pdf). Suggestion for your file names: your last name form number .pdf (e.g. smith45form1.pdf). After saving the forms to your computer you can email them directly to us at services@pawprintinn.com. Thank you.
